Students participating in CMS extracurricular sports must have a valid green physical card on file in the school office before team tryouts.  A physical card signed by the attending physician and a parent remains valid for one year from the date of the physician's signature.  Students that do not have a current physical card on file will not be eligible to tryout or participate on any school team.
